- Instructor Testimony and Biography
- Instructor and Class Submission Form (one for each class)
The paper form (other than the Testimony which should be completed for your application) can be used as a draft before submitting online.

If interested in teaching a Camp, Workshop or Mini-mester, you only need to complete the Statement of Faith, Release of Liability, Instructor Contract and Application, and Camp/Workshop Submission Form.
